Our Aged Care Assessment Services (ACAS) is a service provided for older people and their family or carers to help make decisions about what kind of care is needed when they are no longer able to manage at home without assistance.
ACAS will help you access these services and provide information, referrals and coordination.
There are a range of health services available in the home or in the community designed to support you and your family.
What we do
The ACAS team will assess older people who require access to a range of services to support independence and wellness.
The team is made up of:
- geriatricians (doctors who specialise in the care of older people)
- community health nurses
- social workers
- occupational therapists
The services that are available include:
- Home Care Packages
- Transition Care Program (TCP)
- Residential care
The ACAS team is inclusive of all older people and respectful and accepting of diverse groups within our community. They will arrange an interpreter if needed and will support the involvement of your carer, family member or advocate of you wish.
Who can use our services
ACAS provides services to older people and Aboriginal and Torres Strait Islander (ATSI) people who are over the age of 50.
People who live in the local government areas of Melbourne, Moonee Valley, Moreland and Hume are eligible for assessment by this team.
