Our Community Services can work with you to optimise your health, ability to manage everyday tasks and quality life.
We take a holistic approach and work with you to achieve the goals that are important to you.
We care for people who are leaving hospital and our service is designed to help you remain active and well at home and avoid the need for a hospital admission. For others we ensure you have the extra support you need after a hospital stay.
What we do
We provide the following community services:
- Post Acute Care - short term assistance for people after a hospital stay eg community nursing, home help and physiotherapy.
- Specialist clinics - Outpatient clinics for people with chronic (long-term) conditions such as memory change, pain, falls and continence.
- Community Therapy Services - home and clinic based therapy services (eg occupational therapy, physiotherapy and speech therapy) that help people work towards rehabilitation goals.
- Hospital Admission Risk Program - specialist treatment, care planning, education and support to people who are at risk of hospital admission because of chronic (long-term) or complex health issues.
- Aged Care Assessment Service - assessment, information and advice to older people and their carers to help them make informed decisions regarding individual care needs.
- Hospital in the Home - hospital-level care for patients in their home environment
- Residential In-reach - early treatment to people living in residential aged care facilities, to avoid transferring the resident to hospital
How to access this service
Hospital staff may discuss your eligibility for this service during your visit. Your GP will fax your referral to the hospital.
Once we receive the referral, we will contact you.
If there is a waiting list, we will advise you what to do.
