Our Kidney Care Social Workers provide a unique expertise in assisting patients, carers and families who are living with chronic kidney disease including: pre-dialysis, dialysis and supportive care, renal transplantation and renal transplantation live donor work-up.
The Social Workers provide psychosocial assessment, counselling, education and support to assist with personal, emotional, psychological and practical concerns which may arise as a consequence of health issues. Their aim is to promote independence, to help individuals maintain quality of life and well-being during from the point of diagnosis throughout the entire patient journey.
