All fluids are collected in screw capped containers, to avoid spillage during transit. Fluids should reach the laboratory not later than 1600 hrs during the working week as the preparation time is very lengthy. Do not discard fluid if cytological analysis is required, send as much as possible to the laboratory.

Specimen Requirments: When the brushing is completed, smear the brush onto a labelled glass slide and fixed immediately in 95% alcohol. Label frosted end on the glass slides in lead pencil with the patients name, UR No. and specimen type. Fix the slides immediately in 95% alcohol DO NOT ALLOW THE SMEAR TO DRY. Plastic coplin jars containing 95% alcohol are available from the Cytology laboratory Ext 9342 7853. Immediate fixation is a pre-requisite for all brushing sites - oesaphageal, gastric and bronchial. Air drying artefact caused by a delay in transferring the materal from the brush to the glass slide and subsequently into the fixative makes microscopic diagnostic interpretation difficult. Thus a delay in fixation will yield unsatisfactory or inconclusive results. A post-bronchoscopy sputum should routinely be sent for cytology.

The accuracy of FNA in many situations when applied by experienced and well-trained practitioners can approach that of histopathology in providing an unequivocal diagnosis. The method is applicable to lesions that are easily palpable (skin, breast, thyroid salivary glands, prostate, lymph nodes) or deeper structures with the aid of radiological techniques for internal imaging (lung, abdominal and retroperitoneal organs). A pathologist, together with a scientist are available to perform the FNA procedure and prepare the material when required at O.P. clinics, wards and PMC. The procedure may be performed by the clinician and attended by a scientist from the cytopathology laboratory and requires the provision of a Fine Needle Aspiration kit. To book an FNA - contact cytology. FNA results are routinely available on the next working day or earlier, if urgently required.

All effusions are collected in screw capped containers, to avoid spillage during transit and must be heparinized at the rate of 5 units/ml. Fluids should reach the laboratory not later than 1600 hours during the working week as the preparation time is very lengthy. If effusions are tapped at other times they may be stored in the refrigerator for up to 48 hours without damage to cellular morphology. Cytological analysis requires at least 10ml of fluid and preferably the maximum volume available - up to 2 litres. Do not discard fluid if cytological analysis is required, send as much as possible to the laboratory. In some cases of a long standing effusion the cytological analysis may yield inconclusive results due to degenerative and inflammatory changes to the cells. In these cases a second tap will often yield far better diagnostic material.

This not only makes it softer for easier scraping but also removes most of the degenerate top layer.

Minimum 3 deeply coughed specimens. To be collected on separate days, but if the patient is producing copious sputum, the specimens can all be collected on the same day. Each specimen should reach the laboratory on the date of collection, however, outpatients who are unable to attend the clinic on three separate occasions can collect and refrigerate specimens for one day only, e.g. a patient may take home 3 sterile specimen containers for the weekend and produce two specimens on Sunday, which are refrigerated, ane one on Monday morning before attending the clinic. The specimens are to be collected before meals and before cleaning the teeth with toothpaste, after rinsing the mouth with water.

Discard the overnight or early morning specimens but submit any further fresh, preferably fully voided specimens taken during the day. Please indicate whether the urine sample is voided, catheterised or ileal conduit specimen.
