What we do
Our Home-based service supports older people to recover from their hospital stay, with specialised care delivered in your own home to help you regain function and independence.
Your case manager may arrange:
- allied health services (occupational therapy, physiotherapy, podiatry, speech therapy, dietetics)
- nursing support
- transport to appointments
- help at home such as personal care, shopping or light housework
- social activities
- carer support, such as respite
- specialised equipment

The Aged Care Assessment Service determines whether a person is suited for the program.
How long the program runs
Support is time-limited to a maximum of 12 weeks, though the amount you need will depend on your situation and goals. Most people have access to suitable ongoing care by the end of this period.
If your case manager identifies further therapy goals, they can apply to the Aged Care Assessment Service for a 6-week extension.
Before you're discharged, your case manager will usually contact you and your family, carer or support people to:
- discuss your goals
- work out what help you need
- develop a care plan
Cost
The program is subsidised by the Commonwealth and Victorian governments. You will be asked to contribute a maximum equivalent of 17.5% of the single aged pension.
Fees can be negotiated for those in financial hardship.
