The Virtual Ward allows eligible patients to receive hospital-level treatment from home. Virtual Ward patients attend appointments with their care team via Telehealth video call, or receive at-home visits when needed.

What we do
Each day that you are admitted to this in home service, you will receive:
- a video call from the team (we call this a virtual ward round)
- a call or video call from the nurse, pharmacist or other team members for more in-depth clinical assessment or discussion
- a clear plan for your care for that day
- discussions about when you will be discharged from the service and transferred back to the care of your general practitioner (GP) or another RMH service
- your clinical notes and care plan - these will be available to you via Health Hub (someone will explain how you use this)
- a visit at home by the RMH team if required
- access to 24-hour phone support every day of the week
Who can use our service
People who are admitted to a Royal Melbourne Hospital ward with certain heart conditions and those who have had recent heart surgery may be suitable for this service.
Your hospital team will recommend this service to you if it meets your care needs.
What do we need from you?
- This service is voluntary. We will need your consent to enrol you in the program and to visit you.
- You will need to stay home for your video call, phone calls and home visits.
- If you have medical or other appointments or commitments, please inform your Virtual Ward team before you leave your home.
- You must wear your hospital wrist band while in our service, as it is a requirement for administering treatment.
- As a safety check, we will verbally confirm your identity with each interaction. If you have a wearable device, you will need to keep your phone or the iPad provided with you at all times.
In the pilot stage, we accept internal referrals from RMH wards and services only between 8:30am and 4:30pm.
