Who can use our service
We usually see patients who have been admitted to the RMH with confirmed heart failure. We can also accept patients who have a confirmed diagnosis of heart failure to our Cardiomyopathy/Heart Failure Clinic.

Parkville Connect is a secure web-based portal providing GPs, specialists and other health professionals with information in the Parkville electronic medical record (EMR).
Clinic details
Heart Failure Telehealth Clinic
Patients with a recent admission to the RMH under general medicine for heart failure are seen within 14 days of discharge. In this clinic, we:
- Review the heart failure therapy
- Assess for deterioration in symptoms
- Provide timely intervention to prevent re-admissions
- Provide follow-up education
- Ensure patients are linked in with community-based services
Patients are discharged from this clinic after 10 weeks, and care is returned to your GP, General Medicine or Cardiology as deemed appropriate.
Cardiomyopathy/Heart Failure Clinic
Patients who have a confirmed diagnosis of heart failure, are seen in this clinic for uptitration of medications, assessment for other treatments and interventions, assessment of deterioration in symptoms, education and referral on to the heart transplantation team at The Alfred if deemed necessary.
In all cases, we aim to work closely and communicate well with GPs to ensure ongoing patient care needs are met.

What to bring
Every time you come
Every time you come in for a test, day procedure, surgery or treatment, you should bring:
- Medicare card
- Health Care card (if you have one)
- Concession card (if you have one)
- Adverse drug alert card (if you have one)
- Medications you are currently taking, including any that you have bought without a prescription
- X-ray films, scans, ultrasounds or any other test results you have which are related to your procedure
- Private health insurance card (if you want to use it)
- Aids (glasses, hearing aid, walking frame)
For a clinic appointment
If you come for a clinic appointment, you should also bring:
- Your appointment letter
- Any special items listed on your letter
- Your appointment book (if relevant)
- TAC or WorkCover claim number
For a Telehealth appointment
If you are attending a Telehealth appointment, be sure to have your most recent blood pressure, heart rate and weight measurements ready to tell the doctor.

An important part of this is to have a plan for what you would like, if your symptoms get worse. We work closely with community palliative care services, and can help you make these important decisions.
