What we do
Our Older Adult Community Team provide assessment, treatment, rehabilitation and case management to people with complex and compounding mental health issues related to ageing. We help to prevent unnecessary hospitalisation and minimise the length of stay in acute inpatient facilities.
We work with the individual and their family to assess the severity of their mental illness and recommend an appropriate form of treatment and/or support. In all cases, there is a shared care arrangement with a general practitioner.
Who can use our service
Our service is typically for older adults who are experiencing significant disturbance of thoughts, mood or perception, or severe behavioural or psychological symptoms of dementia.

We see people at our clinics or have the ability to also see people in their own homes, including if their home is in a residential aged care facility.

Referrals
We accept referrals for this service
Call Triage on 1300 874 243 to discuss a new referral.
If your patient is currently seeing a clinician at one of our clinics and you would like to discuss treatment or care plans, contact the appropriate local mental health service directly and ask to speak to the key clinician, Monday to Friday, 9am to 5pm.
For out of hours support, call Triage. If your patient is presenting in an emergency situation, call 000 for an immediate response.
When referring an older adult to our services, Triage is likely to request details of recent clinical investigations and testing (generally within the last 28 days or more recent, depending on the clinical presentation).
