Requests under the Children, Youth and Families Act 2005

Child protection professionals may request access to patient information where required to exercise their powers under the Children, Youth and Families Act 2005 (CYFA).

To request information under the CYFA, email RMHhisFaxRequests@mh.org.au with the patient details and specific clinical information required.

Please ensure the request is signed and dated by a delegate of the Department of Families, Fairness and Housing.
