Transition Care Program
Our Transition Care Program provides care and restorative services to older people who would benefit from some extra support after a hospital stay.
Support can be provided either in the patient’s own home through services and a case manager, or in a residential setting.

Restorative Care Program
Our Restorative Care Program is similar to the Transition Care Program, but it does not have an age limit.
This program is suitable for patients who are near the completion of their rehabilitation program, awaiting home modifications, need assistance during their non-weight bearing period or require complex discharge planning.
Your social worker or member of the healthcare team can answer any questions you have regarding your suitability for this program.
